Yo
Forgot to record my OSCPE experience. Kinda rushed around since it ended. Damn bloody worn out already...
Station 1
Hip examination. Of all the joints.. Explain Trendelenberg Test. Explain Thomas Test. Why I palpated the greater trochanter to which smart me just said dunno without even trying. Shit. Another irritating thing is that the guy wore like 2 layers of tops and 2 layers of bottoms. Had to instruct him like piece by piece which ones he had to take off. Felt that was kinda waste of time. Nvm I finished before the time ended. Should have passed.
Station 2
Blood pressure.
I SCREWED this one up man. I guess at least I figured out how to work the cuff and machine slightly faster than others. I knew I wasn't supporting her arm at heart level the right way but dunno why did not make the effort to correct that. Dunno why I do the stupidest things in exam conditions. Got the BP readin wrong. Nvm that is like 1 point. Asked if I could do it again den the clin dem (dun really like her) was like, u can if u hav enough time but u still have questions to answer. Den she stared at me expextantly. Chose to answer the questions.
1. Name 2 incorrect techniques in which blood pressure can be falsely elevated.
2. Forgot... Dun worry. Got it right.
Attempted to take BP again. So close to finishing. Meaning I still ran out of time. I can only console myself with the knowledge that I was not likely to have gotten n accurate reading again.
Station 3
Rest station. Great thanks. Leave me to hu2 si1 luan4 xiang3.
Station 4
Saw what was DEFINITELY a humerus in front of me den a lower limb prosection next to that.
1. Show the bony landmarks of the distal part of the femur.
Showed every damn thing I could remember. I was staring at the humerus next to me thinking he said the question wrongly at first. Humerus got so many things to show lah! Den a whole bone for me to show. Anyway, I asked like 'on the prosection?' just to make sure.
2. Boy had fracture in distal femur. Why is distal fragment pulled upwards? What group of muscles does this?
I KNOW this lo. Just blanked out. The clin dem was nice. Guided me like idiot-proof guide. Guess Paula is right about if dunno answer just talk it through.
3. What do these muscles do? What other muscles carry out the same action?
I should have gotten this correct. There are only like how many muscles that flex the knee right?
4. Show what position his leg will be like.
Kinda jiggled my leg about abit. Clin dem practically told me the answer. To which I was like, yea that was wat I meant. Wth. See in front I know is flex this question I could still not know how to do. How stupid can I get.
5. Pierced popliteal fossa. Damaged an artery. Why might the muscles in the leg be affected (or smt like that)? Cannot remember the exact question. Quite proud of my answer tho. Pretty well-structured I must say.
6. I tink there was another question about popliteal artery.
7. Gestures to the humerus and asks me if it is the right or left femur. To which I was like, is this not the.... humerus? Left humerus? Response "Well done. I was just teasing you." Kns. The one question I was confident in in the station.
Station 5
Musculo again. I see a bony hand and a prosection of the upper limb. Gasp. Ah well. This was to be expected.
1. Show and name the attachments of the flexor retinaculum (on the bony hand).
I could name the bones. Could show the bones. Just did not know which was the front and back of the hand. Figured the side facing upwards should be the front. Anyway he looked satified. But then again, u never know. Shit.
2. Find the median nerve (on the prosection).
Smt I was confidently teaching my peers (lateral to palmaris longus tendon) but was bewildered at when I stared at the brown mass of stringy things in front of me. I tink I managed to find it in the end. Coz the clin dem nodded. I tink?
3. Find the FDS proximal and distal to the flexor retinaculum.
Ha! Brilliant me pulled the muscle to see wat will move. I tink it was kinda obvious tho. Haha
4. Causes of carpal tunnel syndrome (no prizes for guessing this question)
Blundered my way through. Thank goodness I got it in the end. My fault for brushing TRAMP aside lah. I should SLAP myself for that man. However I got it right. Shan't slap myself then. Haha
5. Why ppl with carpal tunnel syndrome have difficuly holding a pen?
Rattle rattle... Clin dem had to get me to stop.
Station 6
Hip bone and hip X-ray.
1. Position the hip bone on yourself. Right or left hip bone?
I THOUGHT I got it right. However the guy was expressionless. He kept checking and doing things to make sure. so that made me wonder whether I got it wrong.
2. Boy riding bicycle. Fell off. Identify the damage on the X-ray.
Heng I found it after a bit of hard staring. It was blindingly obvious but I was staring at the nitty gritties as usual and missed the big picture at first. The point is I found the fracture.
3. Classify this tgype of fracture.
INTRACAPSULAR FRACTURE. That was like the first thing I said. The guy did not hear and made me tink I got the wrong answer. Den I started describing everything I knew about fractured neck of femur. Den he was like, yea but, ok, what are the classifications of fractures of the femur? Den i just got frustrated den asked is it not just intracapsular and extracapsular? Den he said yea so which is it? Den I repeated my answer again. -_-
Station 7
Presentation
Station 8
1. Girl cycling (again. haha) fell on shoulder. Neck tilted sideways from shoulder. Which part of brachial plexus damaged?
2. 2 groups of muscles supplied by C5.
3. Define myotome.
4. How to test for abduction and supination in patient.
I KNEW how to do it. Dunno why I was so disorganised. May have screwed it up. Shit.
5. Show C5 dermatome on patient.
Poor thing the patient was not bad looking but looked so awkward. Dunno why he sign up in the first place. He did not even kena the more intrusive examinations lah. Ah well. When u are examined by a good-looking person... ah! Understandable...
6. Show C6 dermatome on patient.
7. Wat is the best test to carry out for sensation?
Got this question wrong during exam coz I thought too much -_-
8. Are you an international student?
9. Are you from Malaysia?
Haha just kidding. Small chit chat with the guy.
Station 9
Communication skills
Policeman with paraesthesia in leg and foot. I should have done better for that. Dammit. Usually quite good at talking if I say so myself. Bleahhhhhhhhhhh
Station 10
Rest station. Left me to mull over what had happened so far.
Station 11
Explanation.
Woman with hyperthyroidism. Symptoms of loss of concentration, emotional, palpitations, appetite increase but lose weight, hot and sweat more.
Station 12
Medline. Find review article on woman with elevated thyroxine levels with increased heart rate.
1. State the publicaion details of the article.
2. Why do we use synonyms?
3. State 2 uses of BNF.
4. Give example of synonym.
Somehow I seem to have gotten more questions than others.
Station 13
1. State 2 bony prominances of ankle joint.
Wah lao. Nobody else in the previous days got all these weird musculo questions lah! Dammit. Rambled on... got it.
2. State actions which occur at ankle joint.
3. State 2 ligaments which stabilise the ankle joint.
4. Smt about anterior compartment of leg.
5. Find nerve that innervates this compartment.
Named it as well for good measure. It was glaringly obvious. Was digging around the fibular head for ages. Den I also said out the exact location of the nerve. Thought I found it den went on to the next question.
6. Show muscles which evert the foot.
7. Ok we will go back to the question. Where does the nerve that supplies the anterior compartment start from? Blah blah blah... Den FINALLY I went oh! It is this one! Nice guy. They better pass me.
